HOW TO MAKE A BAN REQUEST!!!!

In order to report someone, you must post a reply stating the following:
Username
Username of person you wish to report
Posts that influenced your decision in banning this person (copy and paste the posts that made you click on the banhammer in the first place)
Reason of reporting
